In addition to completing the standard Form R, all trainees are requested to complete a self-declaration form which can be used by panels as an additional information source to be triangulated with evidence within the trainee’s e-portfolio. This will require trainees to self-assess their progress and provide details of how their training may have been impacted by Covid-19. It includes a “trainee check-in” section which will allow trainees to indicate if they require any additional training or wellbeing support.
