Significant Event Reporting

 

 

Synonyms: critical event audit, critical incident analysis, structured case analysis, facilitated case discussion

Definition

"an event that any member of the team considers to be significant in terms of the care of the patient and the conduct of the practice [organisation]" (Pringle et al.,1995) 

In this case, the organisation is Health Education England, working across North East and North Cumbria. There is no need to report significant events which are unrelated to the functions of the Postgraduate School of Primary Care.

Individual training programmes have a system for documenting and managing significant events. Any events which may have implications within the Primary Care School should be shared with the School via the Business Manager.

All reports on significant events should be submitted to the GP STC who will consider:

a) Any recommendations

b) Any actions, including referral to the HEENE Executive Team

The GP STC will determine any review dates and when the event is to be closed. The QM Co-ordinator will maintain a register of significant events.
